Annual Paid Vacation

The Company provides annual paid vacation to eligible associates.  Vacation is earned on a calendar year basis.  Each January 1, all regular associates working 30 hours per week or more become eligible for vacation based upon the number of January 1sts they have been employed with the Company.  All full time associates will earn vacation based on the following schedule:

New associates begin earning vacation for the current calendar year after completion of the 90-day introductory period.

Health Insurance

Aetna Choice II Plans:  
ABC offers two Aetna Choice II POS plans (Plus and Base) in most locations, both of which have a $15 co-pay for primary physician office visits and a $25 co-pay for specialist visits.  The two plans differ primarily in the amount of the deductibles, co-insurance, out-of-pocket maximums and associate contribution rate. 
No PCP is required.
 


Aetna High-Deductible Health Plan:
A consumer-directed plan that involves high deductibles and out-of-pocket maximums at a low associate contribution. Preventive services, including certain preventive prescriptions, are covered with no deductible. Physician office visits are covered at a co-insurance rate rather than a co-pay and deductibles may apply.


 

 Aetna HMO Plans:  
ABC offers two Aetna HMO plans (Plus and Base) in most locations, both of which have no deductible, a $15 co-pay for primary care office visits and a $25 co-pay for specialist visits.  The two plans differ by the amount of co-insurance paid, the annual out-of-pocket maximums, and the associate contribution rate.  


 Aetna Out of Area Plan:    
 This plan is available only in Aetna service areas where other Aetna plans are not available. $15 co-pay for primary physician office visits and a $25 co-pay for specialist visits.  The plan pays 80% of reasonable and customary charges for most other covered services, after the deductible.  

Other HMO and PPO options may be available in different areas of the country.

Up to 30-day Supply at Retail.

90-day Supply through Mail Order or at Retail using ABC Preferred Customer Pharmacies only.

Vision Plan (VSP)

Life and Disability Insurance

Life Insurance (MetLife):  Company-paid 2 times your annual base salary – maximum of $500,000.

Supplemental Life Insurance:  Select 1, 2, 3, 4 or 5 times your annual salary.  You may also elect coverage for your spouse/domestic partner ($10,000 - $100,000) and dependent children ($1,000-$15,000).  Statement of Health may be required.

Accidental Death & Dismemberment Insurance: Company-paid 2 times your annual base salary–maximum of $500,000.

Supplemental Accidental Death & Dismemberment Insurance:  1 to 10 times base salary for associates - maximum of $500,000.  Family coverage available.

Travel Accident Insurance: Company-paid 2 times your annual base salary - maximum of $500,000, for associates who are required to travel on company business.

Short-Term Disability (Aetna): Non-exempt associates 60% of basic earnings up to $1,000 weekly (26 week maximum).  Exempt associates 100% of basic earnings for first 8 weeks, then 60% for the next 18 weeks, up to $2,000 weekly (26 week maximum).

Long Term Disability (MetLife): After 6 months of disability, plan pays 50% of base pay up to a maximum of $10,000 per month.  Buy-Up Option: Pays 66 2/3% of base pay to a maximum of $15,000 per month; associate contribution required.   Restrictions apply.

 

Other Benefits

Domestic Partner Benefit: Medical, dental, vision and supplemental life insurance coverage available for a same-gender Domestic Partner, including the Partner’s dependent children; affidavit required.

401(k) (Fidelity Investments): Defer 1% - 25% of eligible compensation on a pre-tax basis.  Company match of 100% on the first 3% and 50% on the next 2%, with immediate vesting.  Associate may choose among different investment options.  Additional “Performance Match” when corporation reaches pre-established annual goals; Performance Match vests over five years.

Employee Stock Purchase Plan (ESPP):  Minimum 15% discount on purchase of company stock up to $25,000 in value (1 - 25% of base after-tax pay).

Tuition Reimbursement: Up to $5,250 per calendar year for approved courses and books.

Dress Code: Business casual environment; may vary by location.

Flexible Spending Accounts - Health and Day Care:  Use tax-free money to pay for eligible expenses:  up to $5,000 maximum for health care expenses not covered by medical insurance or HSA and up to $5,000 for day care.

Employee Assistance Program: This confidential, company-paid service offers ABC associates and their dependents assistance and counseling for a wide variety of personal and situational life issues, including marital and family problems, alcoholism, drug dependency, financial and credit concerns, legal issues and questions, emotional problems, stress, child care, elder care, pre-retirement planning, federal tax payer problems, interpersonal conflicts and situational life problems.
